I have created to plans on requesting layouts. The first one is the FREE PLAN where

you could request a layout w/o your name on it. it wiil be published too!
The second one is the PREMIUM PLAN where you could request 5 layouts w/
your name on it even your picture, you can get this plan only by registering on this link:

After picking your plan, pls follow this format below on requesting your layout.

For Free Plans:

Example:

*Note: Pls. be more specific w/ the background description.

For Premium Plans:

- Pls. register at awsurvey first before requesting because you won't be able to provide your AwSurvey refer id if your not registered on AwSurvey.
